STEP 1: Please fill out the form below and you will receive a link within 24 hours to create an account.
STEP 2: After you create an account and log in to the Patient Portal, you will fill out your Primary Info Tab, Insurance Details Tab, Pharmacy Details Tab, and sign the consent forms in order to become a patient. You can then select a provider, location and request an appointment time.
STEP 3: You will recieve confirmation email of your appointment or suggested new appointment time.
STEP 4: You will receive reminder texts and email about your upcoming appointment. Please cancel appointments via the Patient Portal no less than 24 hours prior or you will be charged the no show fee.
